Schedule a sick Appointment 

This feature is ONLY FOR EXISTING PATIENTS.

If your child has NOT been seen in our office please call 904-223-9100 to schedule. 

Schedule Your Appointment in a Few Easy Steps

On the computer

  • Choose the “Schedule my Appointment Through Patient Portal” button
  • Login to your Patient Portal Account (view your health record)
    • In the Patient Portal, under “Appointments” select “New Appointment”
    • Select a convenient Care Center and select “Sick Visit” for a Same Day Appointment
    • If you are looking to schedule an appointment for any other visit type, click “Send Request” to request a future time
    • If you do not see a time that is convenient for you, please call us at 904-223-9100

On the healow App

  • Log into your healow App
  • Click on “Visits” or “Find Appointment”
  • Select a convenient Care Center/Provider and select “Sick Visit” for a Same Day Appointment
    • If you are looking to schedule an appointment for any other visit type (annual visit, follow-up, etc.) click “Send Request” to request a future time
    • If you do not see a time that is convenient for you, please call us at 904-223-9100

IMPORTANT:

    • You must enter a valid phone number to verify your appointment via text message or automated call back
      • Your appointment is not verified until you receive a text or automated call containing a verification code and have entered that code on your booking form. Be sure to keep your appointment booking page open in your browser until you have entered this code
      • If you do not immediately receive a text or automated call or have any questions, call 904-223-9100.
      • Bring your photo ID and primary/secondary insurance cards to your appointment to ensure your claim is processed properly

Please note that online appointments are for SICK non-emergency illnesses or injuries. If you need to schedule a Well Visit, please call our office as this feature is not yet an option. If your condition is an emergency, call 9-1-1 or go to your nearest emergency room.
